Service Description

If using a Therapy Voucher: ****Please forward the approval email from the voucher program, which includes voucher numbers and approved number of sessions prior to booking, otherwise appointment will be canceled**** If using EAP: You are given a set amount of therapy appointments. At the conclusion of your last covered appointment, you will be given the option to continue services with Makinie under Direct Pay, and will need to sign an Electronic Payment Authorization to proceed with services. Required: *All* Teletherapy intake forms must be provided prior to session, or session will be canceled. Accept VeSee chat invite or sign into doxy 5 minutes before intake. During this appointment, I'll answer any questions you might have about the therapeutic process and the intake forms you completed prior to this appointment. If you have any relevant medical documents, please provide it during the appointment (can be done via encrypted email). Please be ready to provide proof of identification prior to your first session

Cancellation Policy

A $150 cancellation fee will be added to your next appointment bill if you do not cancel appointment within 24 hours.
