Service Description

For Insurance Clients: Please be prepared to provide insurance information and show insurance card for review of authorized services. Required: *All* Teletherapy intake forms must be provided prior to session, or session will be canceled. Accept VeSee chat invite or sign into doxy 5 minutes before intake. During this appointment, I'll answer any questions you might have about the therapeutic process and the intake forms you completed prior to this appointment. If you have any relevant medical documents, please provide it during the appointment (can be done via encrypted email). Please be ready to provide proof of identification prior to your first session

Cancellation Policy

A $150 cancellation fee will be added to your next appointment bill if you do not cancel appointment within 24 hours.
